When we last connected with StartUp Health community member Justin Ayars, CEO & Founder of EqualityMD, he was building a direct-to-consumer telehealth platform focused on culturally competent care for LGBTQ patients and their allies. Today, the mission remains rooted in inclusion and trust, but the strategy has evolved in response to a shifting healthcare landscape.

Ayars now describes EqualityMD’s mission as “America’s most affordable and inclusive healthcare solution for employers.” That repositioning reflects both market demand and macroeconomic reality. As employer-sponsored health costs surge and businesses struggle to absorb premium increases, EqualityMD has leaned into a model that helps employers save money while expanding access to care.

A Pre-Insurance Model Built for Employers

EqualityMD operates outside the traditional insurance system. It does not bill insurance and is insulated from payer volatility and shifting policy environments. That independence has become a strategic advantage as employer-sponsored healthcare costs continue to rise.

In response, EqualityMD has leaned into what Justin Ayars now calls a “pre-insurance” solution. Rather than triggering insurance claims for routine care, the platform is designed to resolve most everyday health needs before insurance is ever required.

New research conducted after this interview shows that EqualityMD’s pre-insurance model can resolve up to 80 percent of employees’ diverse health needs without resorting to insurance. By addressing care outside the claims system, EqualityMD can help employers save an estimated 20 to 30 percent in health claim and administrative costs. Employees benefit even more, saving 40 to 70 percent on out-of-pocket expenses such as deductibles, copays, and prescription costs.

The platform now offers tiered SaaS memberships for employees, couples, and families. Each member receives 36-72 virtual visits per year, divided across mental health, primary care, and urgent care. Through its two in-house pharmacies, EqualityMD provides access to more than 1,200 free prescriptions, along with deep discounts at retail pharmacies nationwide.

That distinction matters. Insurance claims are not only triggered by in-person visits. Many telehealth platforms embed their services into employer health plans, meaning virtual visits still generate insurance claims and associated administrative costs. While access increases, overall cost pressure often does not ease.

EqualityMD takes a fundamentally different approach. Because care delivered through its network does not trigger insurance claims, the platform functions as a claim-prevention layer combined with virtual health, wellness, and prescription access. Employers can expand care while actively reducing cost pressure on their health plans.

For small businesses with fewer than 50 employees, this pre-insurance model can serve as a primary healthcare benefit. For mid-sized and larger employers, it complements existing insurance by absorbing routine and preventative needs before insurance is required. Ayars emphasizes that this allows insurance to be used for what it does best, while everyday care is delivered in a more affordable, accessible, and human-centered way.

At its core, the model addresses what Ayars calls “care avoidance.” When healthcare feels confusing, expensive, or unwelcoming, people delay care until problems escalate. By lowering barriers and building trust through culturally competent clinicians, EqualityMD encourages earlier engagement, better outcomes, and lower downstream costs.

Expanding Cultural Competence Beyond One Community

EqualityMD began with a focus on LGBTQ patients, but that lens has broadened. Ayars and his team realized that culturally competent care extends far beyond any single demographic. LGBTQ communities intersect with racial minorities, veterans, rural populations, and many other historically underserved groups.

The underlying mission has not changed, but the scope has expanded. Providers across the network complete EqualityMD’s cultural competence training, ensuring patients feel seen and understood regardless of background. Representation and empathy are not add-ons. They are foundational to rebuilding trust in what many see as a broken insurance-based health system.

Ayars emphasizes that trust changes behavior. When patients feel respected and understood, they are more willing to share information, engage in preventive care, and stay connected to the system. That trust becomes a flywheel, improving outcomes while reducing unnecessary emergency utilization and overall costs.

Turning Patient Journeys Into AI-Powered Insights

Technology is central to EqualityMD’s long-term vision. Through its HIPAA-compliant platform built within the AWS ecosystem, the company captures previously uncollected patient journey data. These insights help fill critical data gaps that large health systems and payers have struggled to address, particularly among historically underserved populations.

Without high-quality data from diverse communities, institutional reforms remain limited. By aggregating anonymized, statistically significant data, EqualityMD can generate AI-powered insights that help larger health entities better understand and engage new markets.

The goal is not simply data monetization. It is systemic reform. Ayars believes that if health companies are given the right insights, they can begin to make institutional changes that encourage proactive engagement rather than emergency-only care. “If we can encourage people to think outside the insurance-only box,” he says, “they’ll find that they have more options, more accessibility, and more freedom to reclaim that autonomy that had been lost for so long.”

That reframing is central to EqualityMD’s broader ambition. Ayars sees the company as having the potential, if properly funded, to challenge how Americans perceive healthcare itself and to prove that affordability, inclusion, and sustainability can coexist.

Building Together to Reshape Healthcare

EqualityMD is actively partnering with small and mid-sized businesses, nonprofits, professional associations, and chambers of commerce seeking affordable healthcare solutions. The company is also raising capital to accelerate growth, with a focus on activating long-standing channel relationships to scale quickly and sustainably.

As a long-time member of the StartUp Health community, Ayars reflects on the importance of building alongside other founders tackling complex health challenges. Healthcare represents one-fifth of the U.S. economy, and no one transforms it alone. Collaboration, shared lessons, and collective resilience matter.

EqualityMD is not positioned as a “vitamin,” as Ayars puts it, but a “pain pill” solving an immediate and urgent need. By combining inclusive care, employer alignment, pharmacy access, and AI-powered insight generation, the company is working to reduce care avoidance and reshape how Americans think about healthcare itself.
